Registration with the Practice

To register with the Practice, please complete the Registration Form, as well as the additional forms - New Patient Health Questionnaire, Communication Needs Form and the Electronic Prescription Nomination form below.  You need to provide two forms of ID - a photo ID and ID to confirm your address in our catchment area (although generally this is not required at present during the Covid-19 pandemic - unless we contact you directly to ask for ID).  If you are on repeat medication please speak to Reception about reauthorising your medication so that it can be dispensed.  You will need to make an appointment for a new patient check with one of our nursing assistants.  You will be asked for information on your medical history, medication, immunisations, lifestyle and general health and family history.  You will also have your blood pressure checked.

When our surgery is open again, please call at Reception to collect registration forms, or, alternatively, please click on the links below to open the forms. When you have completed all of the details on the Registration Form, click on the "Send" button to mail your form to us.  The other forms can be printed, completed, saved by you and then emailed to our email address:

Please ensure that your address is within the Practice boundary - if you are unsure, please check the practice boundary map or contact us.

You will be registered with Abbottswood Medical Centre but you can state which doctor you would normally prefer to see.  All patients, including children, are allocated a named, accountable GP, who is responsible for their overall care at the Practice - this is referred to as your 'Usual GP'.  If you are not aware of your usual GP please do check with any member of staff at any time.

We encourage you to see your usual GP where possible, particularly for ongoing problems, so we can provide you with better continuity of care.  If he/she is fully booked, you can ask for an appointment with another doctor.  We will make reasonable efforts to accommodate requests to change Usual GP.

To enable us to commence your care whilst waiting for your NHS records to arrive, please complete a new patient questionnaire (call at reception or click on the link below to open the form) AND make an appointment to see a nursing assistant.

For adults (over 16's)
